**Child must be 4 years-old on or before September 30, 2016**

*Please bring the following documentation:

Birth Certificate Social Security Card Immunization Record Proof of Income

(3) of the following documents to verify street address: 1.   rent receipt 2.   utility bill 3.   income tax form 4.   voter precinct I.D. 5.   automobile registration 6.   homestead exemption

* Registration cannot be completed if any of the above documents are missing. No exceptions!
